Participants must be current Excel Health customers and must register qualified candidates, which include home health or hospice organizations with an average daily census of 50 or higher that are accredited by CMS. Incomplete candidate forms will not be processed. Each qualified entry will result in a $100 donation made in your and your firm’s honor directly to your choice of a charity from a pre-approved list. Neither you, your firm, nor any of your affiliates may be associated with your chosen charity or have any power to direct the use of the contribution. This program is void where prohibited by law or applicable policy and may be terminated or amended by Excel Health at any time. No purchase is required.
