About Us

Where AI-enabled intelligence meets workflow execution

What began as the industry’s first source of trusted post-acute market insights has evolved into an AI-enabled platform that brings health systems and post-acute providers together through actionable intelligence and connected workflows.

Smarter Decisions. Healthier Outcomes.

Trella Health turns fragmented healthcare data into decisive action.

Fragmented data, disconnected systems, and manual workflows contribute to readmissions, discharge delays, and more than $100 billion in avoidable costs across care transitions.

Trella Health exists to help healthcare organizations solve these challenges.
