By Ellen Knowles | November 10, 2021

Minimize Cost and Improve Outcomes with Better Post-Acute Partnerships

After announcing the FY 2022 Hospice Final Rule, the Centers for Medicaid and Medicare Services (CMS) hosted an online forum to provide details and need-to-know info on the Hospice Quality Reporting Program (HQRP) – specifically addressing the new Hospice Quality Measure Specifications User’s Manual v1.00 (QM User Manual) and the forthcoming changes to two of the program’s four quality metrics (QMs).

In addition to our recent blog post, with key highlights and dates to be aware of, we developed a playbook to help you navigate the changes. The five-step guide will give you the tools to:

  1. Understand the new quality measures
  2. Create a winning game plan
  3.  Get your hospice facility’s team on board
  4. Avoid or understand future penalties
  5.  Staying Competitive

Download this playbook for your 5-step guide to successfully adopting and measuring CMS’ new hospice quality metrics.
